What is Sciatica? Can Chiropractic Care Help Sciatica?

On September 22, 2016, a study was published in the Annals of Vertebral Subluxation Research documenting chiropractic helping a patient with bilateral sciatica. Sciatica is a term that means pain down the sciatic nerves which run on each side from the buttocks down the back of the leg to the foot.

It is actually comprised of fibers from several nerve roots exiting from several levels of the spine. In addition to pain, sciatica can also involve weakness, numbness, or a loss of motor control down the leg. Sciatica usually involves some type of mechanical compression on the sciatic nerve or the sources of the sciatic nerve.

According to the National Institute of Occupational Safety and Health, 70% of the population in industrialized countries will suffer back pain at some time. It is estimated that between 13 to 40% of the public will suffer from sciatica at some time in their lives. Annually, it is estimated that between 1 and 5% of the population deals with this issue in a given year.

In this case, a 77-year-old man went to the chiropractor. The man was suffering from bilateral sciatica which started a year earlier after a car accident. The pain was so severe that the man was unable to walk more than four steps. In addition to the sciatica, the patient was suffering from diabetes, depression, and constipation.

A chiropractic examination was performed on the man which included range of motion, palpation, infrared paraspinal thermal scans, spinal x-rays, and motion x-rays. The man’s range of motion was limited due to pain, and palpation showed areas of tenderness over the pelvis at the origin of the sciatic nerve. From the examination and x-ray findings, a determination was made that subluxations were present in the patient’s spine.

Specific chiropractic adjustments were given to address the subluxations found to be present. After a month and a half, totaling eleven visits, a re-examination was performed. At that time, the man reported that his sciatica had been significantly reduced. He also reported that he was able to walk for a longer distance than before. His range of motion was also greatly improved.

In their conclusion the authors wrote, “Sciatica and other lumbar radicular syndromes are painful conditions that can cause significant disability to a patient.” They also noted that specific chiropractic care for the reduction of vertebral subluxations can have a positive effect on bilateral sciatica symptomatology.

Pregnancy-Related Lumbopelvic Pain Improved with Chiropractic

What Can I Do For Pregnancy Pains? Do Chiropractors See Pregnant Patients?

The June 2016 issue of the Journal of Chiropractic Medicine published a case study involving a pregnant woman suffering from pregnancy-related lumbopelvic pain (PR LPP) being helped by chiropractic care.

Those who do not gain relief are more likely to continue to have pain after their pregnancy. A previous randomized chiropractic clinical trial showed that chiropractic care given in conjunction with the normal obstetric care was more effective in eliminating pregnancy-related lumbopelvic pain than just obstetric care by itself.

In this case, a 35-year-old pregnant woman presented herself for chiropractic care. She was suffering with moderate pregnancy-related lumbopelvic pain and leg pain. The pain was over both pelvic bones across her lower back. She also reported having an “uncomfortable” tension in her neck and shoulders.

She rated her back and leg pain as a 7 out of 10, with 10 being the worst. The pain started at about the 20th week of her pregnancy and was now radiating down her leg. She reported that standing or sitting for more than 30 minutes, or walking for more than 10 minutes, made her condition worse.

Her history noted that this pregnancy was the result of her fourth in vitro fertilization in a time period of less than 5 years. A previous attempt resulted in the birth of her son. However, two additional attempts to become pregnant resulted in complications and terminated pregnancies.

After an examination, chiropractic care was initiated. The woman was also given some stretching exercises and advised to slowly increase the amount of walking she was doing. Within one week of starting chiropractic care, the woman reported a reduction in the severity and duration of her low back and leg pain. She noted that she was able to sit and walk for longer periods of time.

After 13 visits, she was able to walk or stand for longer than 30 minutes, and sit and travel for more than one hour in the car. This made her daily activities much easier and she reported a decrease in her stress and anxiety. Her overall pain rating dropped from a 7 to a 2 out of 10.

What is a Synovial Cyst? Can Chiropractic Care Help Radiating Low Back Pain?

The June 2016 issue of the Chiropractic Journal of Australia published a case study documenting the case of an elderly woman who had been suffering with pain in her lower back and leg being helped with chiropractic. This case was unique due to the woman having an MRI confirming the presence of a synovial cyst in her lower spine.

Lower Back Pain with Radiating Pain Down Leg into Foot Helped with Chiropractic - Austin TX Chiropractor car crash truck semi accident careA synovial cyst is an uncommon occurrence where a cyst or soft tissue lesion forms inside the spinal column just outside the spinal cord. It is usually found in the lower back and is associated with lower back pain with radiating pain into a leg. It is commonly seen with some amount of spinal degeneration, and is more common in elderly patients. When discovered, the most common form of medical care is surgery.

In this case, a retired 76-year-old woman was suffering for the previous 6 months from pain in her buttocks down her right thigh, down her leg into her right foot. She also reported abnormal sensations in her right leg. With ten being the worst, the woman reported that pain as an eight. Standing and walking made her worse while sitting afforded some minimal relief.

A physical examination was performed which included reflexes and observation. An MRI was performed prior to chiropractic care and showed the presence of a synovial cyst along with a mild amount of spinal degeneration in the lower spine.

Initial chiropractic care was begun on the woman followed by supportive care. The study reports that after only a short course of care, the patient reported a complete resolution of her symptoms.

During a six-month follow-up, it was noted that the patient was still symptom free. At this point, a second MRI was performed to check the status of the cyst. The MRI showed that, although the patient was free from the pain, the cyst did not seem to have changed.

The authors noted that this case puts to question whether or not a synovial cyst is the cause of the pain. They stated, “This case report shows that although symptomatic resolution occurred following conservative care, the synovial cyst may not have resolved. This leaves open the question concerning the mechanism of symptom generation in these cases.” They noted the uniqueness of this case and the findings by adding, “This is the first reported case of MR imaging appearances of a lumbar facet synovial cyst pre- and post- successful conservative management resulting in symptomatic resolution.”
