What is Sandifer Syndrome? Can Chiropractic Help With Digestive and Neurological Problems?

The Journal of Pediatric, Maternal & Family Health published a case study on April 14, 2016, documenting the case of a baby girl who was suffering with a variety of digestive and neurological problems being helped by chiropractic. The baby had been diagnosed medically with Sandifer syndrome since birth.

It is believed that these spasms may actually be seizures and appear as sudden extensions of the head and neck where the head may be twisted from side to side and the upper part of the trunk bent from side to side.

In this case, a 12-pound, 11-week-old baby girl suffering from reflux and digestive problems, possible seizures, and possible colic, was brought to the chiropractor. She had been diagnosed by a pediatric neurologist and pediatric GI specialist with Sandifer syndrome. Her symptoms included explosive bowel movements since birth, but she started to become constipated, going 3-5 days without a bowel movement. It was reported that the infant screamed uncontrollably during her first week of life.

The infant girl also experienced regular hiccups, vomiting, and difficulty lying flat which would cause her hands to shake, eyes bounce, face to become red, and she would scream. Due to the severity of her suffering, the baby was placed on the powerful medication Zantac twice per day.

A chiropractic examination with spinal palpation was performed. Vertebral subluxations were found and specific adjustments were done as needed. By the seventh visit, the study reports that the baby showed more frequent bowel movements, decreased reflux, decreased gagging, decreased spitting up, and was able to lay flat again.

As care continued, by the 10th visit it was noted that the patient began sleeping in her crib for six hours a night and was having bowel movements daily. On the 12th visit, the baby’s mother was asked to fill out an evaluation form to report on the progress of her baby girl. In the evaluation, she noted that her daughter was less congested, had no more episodes of shaking, was better during tummy time, showed less fighting when eating, had better sleeping, and more frequent bowel movements. The baby girl also exhibited more energy, and an overall increase in playfulness.

What is Polypharmacy? Can Chiropractic Care Reduce My Need For Medication?

The headline above comes from a March 21, 2016, Reuters news story published in response to a pre-released study on this subject published in the April 2016 issue of JAMA Internal Medicine. This article, and several others on this same subject, reported on the growing usage of medication by the increasing elderly population.

This was up from 84.1% in 2005-2006. Even more alarming was that the study showed that people using five or more prescription medications increased from 30.6% to 35.8% in the same five year period.

The Reuters article begins by noting that people using multiple medications, known as polypharmacy, was associated with an increase in major drug interactions in older Americans. Dr. Dima M. Qato, the study’s lead author from the University of Illinois at Chicago, commented, “That’s a concern from a public health standpoint, because it’s getting worse.”

The study showed that because of the increase in polypharmacy, the percentage of people at risk for serious or even life-threatening drug interaction has gone from 8 percent in 2005-2006 to 15 percent in 2011-2012.

Dr. Qato also stated in a CBS interview on this issue, “Many of these potentially deadly drug interactions involve prescription and non-prescription medications and supplements that are not commonly used, but are increasingly being used by older adults. While it is not known how many older adults in the U.S. die of drug interactions, the risk seems to be growing and public awareness is lacking.”

“It is time to take the next leap forward. We need to create systems that support an ongoing process of monitoring medications,” said Michael A. Steinman, M.D., of the University of California, San Francisco, in an accompanying editorial. “Such systems would help us periodically assess the benefits, harms and ongoing need for each of a patient’s medications, as well as the reasonableness of the medication regimen as a whole. These systems could also help physicians with deprescribing, for example by supporting gradual down-titration of a medication and monitoring patients for adverse drug withdrawal reactions after a drug is stopped.”

“As much as we need to support reforms in medication monitoring, we seriously need to examine the culture of taking so many medications,” says Robert Braile D.C. chairman of the International Chiropractors Association Public Relations Committee. “Americans represent about 5% of the world’s population, yet we consume about 50% of the prescription medication in the world. This, coupled with the fact that the U.S. ranks far below many other nations in almost all health statistics except spending, should lead us to question whether or not the culture of a pill for every ill is a healthy and prudent direction.”

A study published in the Journal of Manipulative and Physiological Therapeutics (JMPT) on March 28, 2016, showed that when there was more spent on chiropractic services, or more chiropractors in an area, there was overall less opioid usage.

According to the CDC, nearly 2 million Americans abused or were dependant on prescription opioids in 2014. Between 1999 and 2014, 165,000 people died from prescription opioids. In response to this problem, the CDC issued their new “Guideline for Prescribing Opioids for Chronic Pain.”

The purpose of the JMPT study was twofold: to see if either the ratio of chiropractors to the general public, or the amount spent on chiropractic services had a correlation with opioid use among younger, disabled Medicare beneficiaries.

This study looked at the number of chiropractors per 1,000 Medicare beneficiaries as well as per-capita spending for chiropractic care on Medicare beneficiaries in the year 2011. The results showed that more chiropractors, or more spending on chiropractic services was associated with a decrease in the usage of opioid medication in younger, disabled Medicare beneficiaries.

“When people choose chiropractic, there is a less chance that they will turn to opioid pain medications,” states Dr. George Curry, president of the International Chiropractors Association. “Chiropractic has always offered drug free choice to a population that is being continually bombarded with pharmaceutical advertising.”

The Foundation for Chiropractic Progress (F4CP) issued a release on the problem of opioid usage on April 4, 2016, which stated, “With 72 percent of doctors prescribing opioids for back pain and 67 percent prescribing these potentially addictive drugs for chronic joint pain, the F4CP points to the value of evidence-based, drug-free chiropractic care as a safe and effective alternative for pain management often resulting from back, neck, musculoskeletal conditions and headaches.”

The F4CP release continued, “Given the high prevalence of these musculoskeletal conditions — an estimated 126.6 million Americans (one in two adults) validated by a March 2016 report from the United States Bone and Joint Initiative (USBJI).” the care provided by doctors of chiropractic (DCs) becomes even more important.”

Is Chiropractic Care Cost Effective? By Seeing A Chiropractor Can I Avoid Surgery?

The January 2016 issue of the Journal of Manipulative and Physiological Therapeutics published a study titled, The Association Between Use of Chiropractic Care and Costs of Care Among Older Medicare Patients With Chronic Low Back Pain and Multiple Comorbidities. The study showed that patients on Medicare who used chiropractic care decreased overall costs compared to those who did not seek chiropractic.

Researchers conducted a retrospective review of services for chiropractic care on Medicare patients between the years of 2006 and 2012.

The study reported the results of 72,326 Medicare beneficiaries who were 66 years or older and had chiropractic services that were covered by Medicare. Medicare currently only pays for chiropractic care for adjustments of subluxations.

The study compared the cost of total care for patients who received just chiropractic, with those who received chiropractic and medical care, and those who only received medical care without chiropractic. The study also only reviewed those who were seen for chronic lower back problems in order to have consistency in comparing results.

The results of the study showed that those patients who only received chiropractic care had the lowest costs for the care for their problem. This highest cost of care were those who only received medical care. Those who received chiropractic also had a much lower chance of having spinal surgery. Additionally, those who only received chiropractic care had a better outcome in a shorter period of time.

In response to this study, Dr George Curry, president of the International Chiropractors Association commented, “This study highlights the fact that those who receive chiropractic save money on healthcare expenditures. Although this study specifically looked at Medicare patients who were suffering from lower back issues, it should not be a far leap to extrapolate this out to a number of health issues that people seek chiropractic for. Chiropractic care should be viewed as a real solution to the spiraling costs of medical care.”

The Centers for Disease Control and Prevention issued a release on March 3, 2016, with the above headline. The release notes that antibiotic resistant bacteria, commonly called superbugs, are now causing 1 in 7 healthcare associated infections, (HAI).

The release notes that HIAs are dangerous because they are resistant to antibiotics and very difficult to treat. The release states, “HAIs are commonly caused by antibiotic-resistant bacteria, which may lead to sepsis or death.”

A Fox News story on the same issue noted that the CDC reported that on any given day as many as 1 in 25 patients have an infection they picked up while in the hospital. Estimates are that for some procedures, the infection rate is as high as 1 in 4 patients.

The CDC release noted three areas that need to be addressed to help combat HIAs. These are, ” 1) prevent infections related to surgery or placement of a catheter, 2) prevent spread of bacteria between patients, and 3) improve antibiotic use.”

The discussion of the emergence of antibiotic resistant bacteria has long centered around the overuse of antibiotics in medical care and food that has fostered the development of bacteria that are no longer treatable by these drugs.

Resolution of Fatigue, Malaise and Depression with Chiropractic

Can Chiropractic Care Help Depression? How Does Chiropractic Help Fatigue And Lack Of Energy?

The February 29, 2016, edition of the Annals of Vertebral Subluxation Research published a case study showing chiropractic helping a man suffering from general tiredness, fatigue, loss of energy and depression. According to the study authors, generalized fatigue and lack of energy are a common disorder accounting for about 5% of all adult visits to primary care physicians.

His chief complaint was that he was suffering with general tiredness and loss of energy. He also reported having occasional headaches and acid reflux.

He described his condition as a generalized malaise and lethargy, with occasional feelings of sadness and depression. The patient did not recall any specific incident that triggered his problems but noted that he has had these symptoms for over 12 years.

He reported that there was not a regular time or day in which he felt his symptoms were worse, but he did report that it was difficult to wake up and “get going” in the morning. Although he was able to conduct the normal activities needed to take care of himself and his family, he felt he lacked the energy to engage in the additional activities he would like to do. He did report that strenuous activity or exercise seemed to give him more energy. The patient’s family also noted his lack of energy and depression. He was not under medical care, or taking any medications for his condition.

A chiropractic examination of the patient consisted of a history, a postural inspection, basic physical examination, spinal palpation, spinal thermal readings (heat), and spinal x-rays. The palpation examination showed areas of increased spinal sensitivity and edema with taut fibers. The thermal examination showed areas of heat changes along the patient’s spine. Postural irregularities were observed and there was a slight reduction in normal spinal range of motion. The spinal x-rays confirmed spinal malpositioning.

It was determined that vertebral subluxations were present resulting in neurological dysfunction throughout the patient’s body. From this, a series of specific chiropractic adjustments were performed to address the subluxations that were found. The results of the care were measured by re-examination as well as self reports from both the patient and his family.

As his chiropractic care progressed, the patient reported an increase in energy and positive attitude. His family also reported that he was able to handle stressful situations much better. The study noted that, “His wife confirmed that he was able to complete more responsibilities at home during the week and on weekends with less difficulty and less complaints about low energy or tiredness.” The patient also began engaging in family outings which included fishing and hiking.

In their conclusion, the authors noted that the chiropractic care and management of this case focused on the detection and reduction of vertebral subluxations found in this patient. They commented, “This case study demonstrates the efficacy of subluxation-based chiropractic care for the management of fatigue and malaise in a single patient.”

CDC Study Says Most People Seeing Chiropractors Are Paying for Care

A January 2016 report released by the Centers for Disease Control and Prevention’s National Center for Health Statistics showed that even though insurance affects how people purchase healthcare, more people than ever are choosing to pay for their chiropractic care. The report was based upon the National Health Interview Survey (NHIS) taken in the years 2002 and 2012.

They checked the usage of these three services specifically in the years 2002 and 2012. Researchers also looked to see if those surveyed had insurance that covered those services, and if insurance coverage affected the amount of usage of those services.

The results showed that there was increased utilization in all three services from 2002 compared to ten years later in 2012. The rates of utilization in 2002 were: acupuncture – 1.1%, massage – 5.0%, and chiropractic – 7.5%. These figures all increased ten years later to: acupuncture – 1.5%, massage – 6.9%, and chiropractic – 8.3%.

When examining who had insurance coverage, it was noted that the group with insurance coverage did not show an increase in utilization, while those without insurance, who would have to pay for their care, showed a statistically significant increase in utilization. This means that having insurance did not cause more people to seek out these three services. The study stated, “Although increases were observed in the percentage of adults who saw a practitioner for acupuncture, chiropractic, or massage therapy and did not have health insurance coverage for these visits, no changes were observed among those who saw a practitioner and had coverage for these complementary health approaches.”

For chiropractic, the study noted that 18.7% of those who sought those services had full insurance coverage. Partial coverage for chiropractic made up 41.4% of those who went to the chiropractor, while 39.9% had no chiropractic insurance coverage at all. Both acupuncture and massage had considerably less insurance coverage than chiropractic.

In their summary the authors of the CDC study noted, “Increased use of acupuncture, chiropractic, and massage therapy between 2002 and 2012 was previously noted. There was a significant increase in the percentage of adults who saw a practitioner for acupuncture, chiropractic, or massage therapy but did not have health insurance coverage for these approaches. No change in use was observed among those with insurance coverage. These data suggest that consumers are increasingly willing to pay out of pocket for the use of acupuncture, chiropractic, and massage.”

Study Suggests Most Subluxations Do Not Cause Symptoms

A study published in the January 28, 2016, issue of the Annals of Vertebral Subluxation Research showed that most subluxations do not cause symptoms of any kind. The study was conducted in Germany using a survey of chiropractic offices.

Study Suggests Most Subluxations Do Not Cause Symptoms - Austin TX Chiropractor best yelp google reviewsThe definition of subluxation according to the Association of Chiropractic Colleges is: “A subluxation is a complex of functional and/or structural and/or pathological articular changes that compromise neural integrity, and may influence organ system function and general health.” In simpler terms, a subluxation (a.k.a. Vertebral Subluxation) is when one or more of the bones of the spine (vertebrae) move out of position and create pressure on, or irritate, spinal nerves. Spinal nerves are the nerves that come out from between each of the bones in the spine. This pressure or irritation on the nerves then causes those nerves to malfunction and interfere with the signals traveling over those nerves.

The study reports that the International Federation of Chiropractors and Organizations (IFCO) considers subluxations by themselves to be sufficient reason for chiropractic care. “The correction of vertebral subluxation is appropriate for anyone exhibiting evidence of its existence regardless of the presence or absence of symptoms and/or disease. Therefore, the determination of the presence of vertebral subluxation stands as a sole rationale for care.” The Council of Chiropractic Education also notes, “Vertebral subluxation may be asymptomatic, yet still exert various physiological effects.”

In this study, a survey was sent to 25 German chiropractic offices asking them several questions. Nineteen offices responded to the questions including, “How many of the patients that you treat every day have asymptomatic subluxations?” The intention of this question was not to look for patients that had no symptoms at all, but rather to look for patients with subluxations that were not causing the patient any symptoms. The survey also asked the offices how many patients they saw each week.

The results of the survey showed that, on average, 93.73% of patients who went to the chiropractor had some asymptomatic subluxations. These subluxations could have been in addition to subluxations that were causing the patient symptoms. The offices surveyed represented a variety in the number of patients that were seen with the average being 172 patient visits per week.

In their conclusion, the authors noted the importance of detecting subluxations that may not show symptoms by stating, “From the analysis of the collected data it appears that asymptomatic subluxations that are not consciously perceived are common among patients presenting in chiropractic offices in Germany. This lends support to the widespread belief among chiropractors that people should be checked for subluxations regardless of conscious symptoms being present.”

Infant’s Constipation Resolved After Chiropractic

The Journal of Pediatric, Maternal & Family Health published a case study on February 1, 2016, documenting the case of a baby boy suffering from constipation being helped by chiropractic. The study reports that constipation is a common pediatric problem affecting about 3% of children.

Their website describes how to detect painful movement in an infant. “Infants experiencing painful bowel movements might arch their backs, tighten their buttocks or cry. Keep in mind that infants have weak abdominal muscles and often strain during bowel movements.”

The authors of the study note that most medical approaches to constipation are not very effective. They explain, “When one considers the perspective that approximately 30% of children beyond puberty continue to struggle with symptoms of constipation, new therapeutic strategies are necessary thus making alternative therapies an attractive care option for children suffering from the disorder.”

In this case, an 8-month-old baby boy was brought to the chiropractor by his mother to see if he could be helped with his constipation. The infant’s mother noted that her son had not had a bowel movement for five days and was in obvious pain and discomfort. She observed that her son would strain for a bowel movement without success. The baby was given fluids and not given any medications.

A chiropractic examination consisting of heat readings, static and motion palpation was performed. It was determined that subluxations were present, and with the consent of the mother, specific corrective chiropractic adjustments were begun.

The effects of the chiropractic adjustment were immediate. The case study reports, “As the mother and son were in their car to leave the clinic parking lot, the patient proceeded to have a very large bowel movement while sitting in his car seat. The mother phoned the clinic two hours later to inform that the patient had proceeded to have an additional two bowel movements since they had left the chiropractic clinic.” The mother also reported that her infant son’s disposition had greatly improved.

In their conclusion the authors wrote, “We described the successful chiropractic care of an infant suffering from constipation. In addition to resolving the child’s presenting complaint, the patient’s quality of life improved significantly as reported by his mother.”

Erb’s-Duchenne Palsy Resolved with Chiropractic: Case Study

In the January 21, 2016, issue of the Journal of Pediatric, Maternal & Family Health is a published case study of a baby girl suffering with Erb’s-Duchenne Palsy who completely recovered under chiropractic care. Erb’s-Duchenne Palsy is also known as Brachial plexus palsy or simply Erb’s Palsy.

It is more commonly seen in newborns (neonates) and is often the result of a difficult delivery.”

The authors of the study report that most of the cases of Erb’s Palsy are medically managed with either physiotherapy or surgery. The results of this care show that 30% recovered with minimal defects by six months of age, while 55% had moderate residual deficits by 12 months, and 15% had long-term significant handicaps.

In this case, a 5-month-old infant diagnosed with Erb’s Palsy was brought in for chiropractic evaluation. The mother reported a difficult delivery lasting 12 hours. She mentioned that during delivery the head and shoulders “got stuck.” Therefore, a vacuum extraction was performed. Immediately after birth, it was noticed that the infant had a diminished range of motion in her right arm and a diagnosis of Erb’s Palsy was made, and physiotherapy recommended. The therapy was not successful in helping the infant.

The chiropractic examination confirmed the severe lack of motion and care was initiated for correction of subluxations found in the infant girl’s spine. Specially modified chiropractic adjustments were used due to the infant’s size, age, and condition.

On the second visit, a slight improvement was noted in the range of motion of the infant’s arm. By the third visit, the change was even more noticeable and was confirmed by the physiotherapist who pointed out the improvement to the parents. By the seventh visit, the infant’s range of motion was much improved and the infant was observed using her right arm to reach for toys. On the ninth visit, the infant girl no longer had any noticeable difference in the range of motion between her right and left arm and was able to use both normally. A follow-up examination weeks later continued to show total recovery from her Erb’s Palsy.

In their conclusion the researchers stated, “This case chronicles the care and subsequent resolution of Erb-Duchenne Palsy in a five-month-old female with chiropractic care. This case report suggests that chiropractic care may benefit infants with Erb-Duchenne Palsy.”
